SICKKIDS 150th 50/50 RAFFLE AND SICKKIDS BIRTHDAY BONUS DRAW - RULES AND REGULATIONS

General: By purchasing a ticket in the SickKids 150th 50/50 Raffle and the SickKids Birthday Bonus Draw, the purchaser acknowledges that they have read, understood, and agreed to these rules. All inquiries regarding the conduct, chance of winning, prizing or procedures of lotteries may be made to The Hospital for Sick Children Foundation (“SickKids Foundation”) prior to the purchase of a ticket. All orders will be processed and fulfilled by SickKids Foundation via an electronic raffle management system registered with the Alcohol and Gaming Commission of Ontario. In the event of any discrepancy, the records of the electronic raffle management system shall be deemed correct and final. The liability of SickKids Foundation shall be limited to the purchase price of the SickKids 150th 50/50 Raffle ticket(s).

Eligibility: By purchasing a ticket in one or both of the aforementioned SickKids Foundation raffles, the purchaser confirms that they are at least 18 years of age and located in the province of Ontario. Only tickets that have been paid for and have been verified as paid for are eligible for the respective draws. The following persons are prohibited from purchasing tickets or obtaining benefit from the prize offering: SickKids Foundation staff and Board of Directors; The Hospital for Sick Children Executive Team and Board of Trustees; employees of registered gaming suppliers; and members of the same household of each of the above.

Tickets: Ticket purchasers must be at least 18 years of age and tickets cannot be ordered in the name of a minor. Tickets must be ordered from the Province of Ontario, and may be ordered online at sickkids5050.ca. Venue purchases via handheld terminals may be available at The Hospital for Sick Children (555 University Avenue, Toronto, Ontario M5G 1X8) and at the event commemorating the 150th anniversary of The Hospital for Sick Children, on June 7, 2025 (the “Event”). This Event will be held at Nathan Phillips Square, 100 Queen Street West, Toronto, Ontario M5H 2N3. Tickets will be available until midnight, June 8, 2025 or, in the case of the SickKids Birthday Bonus Draw, while supplies last. Purchasers should receive their tickets within a few minutes of placing an order. All tickets in a bundle pack must contain the same name and electronic mailing address. Only one method of payment will be accepted with any ticket order and purchases are not tax deductible. The official emailed SickKids 150th 50/50 Raffle ticket is a receipt of purchase. It is the responsibility of the ticket purchaser to ensure that they receive their tickets, and to contact SickKids Foundation in the event they do not receive their tickets. However, however, purchasers do not need their tickets in hand to be included in the draws. No refunds will be issued after the commencement of the draws. Prizes/Draws: Prizes will be awarded as listed. All prizes will be issued as cash and must be accepted as awarded. There are no restrictions on how to use the prize proceeds. All draws will be conducted at the offices of SickKids Foundation. All draws will be made using Random Number Generator (RNG) software provided by BUMP5050 Worldwide, which will randomly select one (1) potential winner from among all eligible ticket numbers for each of the prize draws. All draw times are approximate. Winning ticket numbers are eligible for all subsequent prize draws. Please allow four weeks to receive prize cheques. Unclaimed prizes will be secured for a period of six (6) months following the close of the applicable SickKids lottery. Unclaimed prizes will be reported to, and a charitable allocation will be approved by, the Alcohol and Gaming Commission of Ontario.

Prize Winner Notification: Prizes will be awarded to the purchaser listed on the ticket. All winning ticket numbers will be posted at sickkids5050.ca. Winners will be notified via email as to what they have won and how to claim their prize. Prize winners, including the SickKids Birthday Bonus Draw, will be required to provide identification verifying an age of 18 years or older.

Privacy: By claiming a prize, the winner consents to the use of their likeness/photograph, first and last name, ticket number, city/town of residence and the audio/video recording (or written transcription) of the winner notification telephone call, for any publicity purposes by SickKids Foundation. SickKids Foundation respects the privacy of its lottery supporters; we do not sell, rent, or trade our supporter lists. The information we collect is used to process lottery ticket purchases, keep our supporters informed about the activities of The Hospital for Sick Children and SickKids Foundation, and ask for their support for our mission to improve children’s health. Limitation of Liability: By purchasing a ticket, purchaser releases and holds harmless SickKids Foundation, The Hospital for Sick Children, and all of their respective directors, officers, members, employees, agents, successors and assigns from any and all liability for any injuries, loss or damage of any kind arising from or in connection with the SickKids 150th 50/50 Raffle and the SickKids Birthday Bonus Draw or any prize won.

Governing Law: These rules and the rights and obligations and relations of the parties hereto shall be governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein. Each party hereto does hereby attorn to the jurisdiction of the Courts of the Province of Ontario.

SICKKIDS 150th 50/50 RAFFLE

Unlimited number of SickKids 150th 50/50 Raffle tickets are available for purchase. Ticket bundles and prices are as follows: three (3) for $50; ten for $100 and 50 for $150. A combination pack is available for $250, consisting of 150 main SickKids 150th 50/50 Raffle tickets and 150 Birthday Bonus Draw tickets. Sales revenue will be allocated as follows: $175 to the 50/50 raffle and $75 to the bonus draw. One ticket will be issued for each order placed, listing all ticket numbers issued. Ticket numbers will be randomly assigned through the electronic raffle system.

Prizes/Draws: The main prize is a single cash prize, consisting of 50 percent of all revenue generated from ticket sales of the SickKids 150th 50/50 Raffle. The guaranteed minimum cash prize is $150,000 and there is no maximum amount. The first Early Bird Prize draws will be conducted at 10:00 a.m EST. on April 28, 2025, consisting of fifteen (15) draws for $1,000 cash. To be eligible, tickets must be ordered by midnight, April 25, 2025. The second Early Bird Prize draws will be conducted at 10:00 a.m. EST on May 12, 2025, consisting of five (5) draws for $10,000 cash. To be eligible, tickets must be ordered by midnight, May 9, 2025. The third Early Bird Prize draw will be conducted at 10:00 a.m. EST on May 26, 2025, consisting of one (1) draw for $50,000 cash. To be eligible, tickets must be purchased by midnight, May 23, 2025. The final Jackpot Prize draw will be conducted at 10:00 a.m. EST on June 11, 2025. To be eligible, tickets must be purchased by midnight, June 8, 2025.

Prize Winner Notification: The first Early Bird Prize winners will be posted at sickkids5050.ca by 1:00 p.m. EST on April 28, 2025. The second Early Bird Prize winners will be posted at sickkids5050.ca by 1:00 p.m. EST on May 12, 2025. The third Early Bird Prize winner will be posted at sickkids5050.ca by 1:00 p.m. EST on May 26, 2025. The Jackpot Prize winner will be posted by 1:00 p.m. EST on June 11, 2025.  Prizes will be awarded to the purchaser listed on the ticket. SICKKIDS BIRTHDAY BONUS DRAW

One prize will be awarded in the SickKids Birthday Bonus Draw. The value of this single cash prize is $180,000 provided in one lump sum. To be eligible for the SickKids Birthday Bonus Draw, tickets must be purchased by midnight on June 8, 2025. SickKids Birthday Bonus Draw tickets can only be ordered in conjunction with the SickKids 150th 50/50 Raffle. The single SickKids Birthday Bonus Draw prize draw will be conducted on June 11, 2025, following the SickKids 150th 50/50 Raffle Jackpot Prize draw and published on June 11, 2025. SickKids Birthday Bonus Draw tickets are $10 each; five (5) for $25; and 50 for $50. A combination pack is available for $250, consisting of 150 main SickKids 150th 50/50 Raffle tickets and 150 Birthday Bonus Draw tickets. Sales revenue will be allocated as follows: $175 to the 50/50 raffle and $75 to the bonus draw. Ticket numbers will be randomly assigned through the electronic raffle system. There are only 400,000 tickets available for sale for the SickKids Birthday Bonus Draw and there are a limited number of single tickets (25,000) and 5-pack tickets (60,000). If all SickKids 150th 50/50 Raffle tickets held by a ticket holder become invalid, all SickKids 2025 Birthday Bonus Draw tickets associated with the ticket holder will also become invalid.

Prize Winner Notification: Winning ticket number will be posted at sickkids5050.ca by 1:00 p.m. EST on June 11, 2025. Prizes will be awarded to the purchaser listed on the ticket. Winner will be notified via email that they have won this prize and how to claim this prize. Prize winner will be required to provide identification verifying an age of 18 years or older.
